Identifying Suitable Acute Care Rehab candidates

What assessment or screening tools are other acute care facilities using to

identify those patients who will be good candidates for Rehab vs skilled?

We have been looking at the University of Rochester Acute Care Evaluation but

can't find research to support validity.

Also looking at Gait Velocity but that doesn't address patients who are

non-ambulatory.

Barthel and Katz would require multi-disciplinary involvement, so have some

compliance concerns.

Ideas and thoughts greatly appreciated.
