Electronic Clinical Questionnaire Softwares

Dear Group,

Does anyone have experience using computer software for clinical questionnaires,

such as the oswestry pain scale, DASH, neck disability index, etc? Any

recommendations?

Alice,

The NDI, ODI and DASH are copyright protected and most authors are willing to

grant you permission for clinical use.

You'll have to format your templates yourself - a .pdf can be set up so the

patient can enter fields directly.

Try to push the data collection to staff and to the patient in the waiting room.

A tablet PC or desktop set up in the waiting room allows the patient to enter

data. Even e-mail options are available now.

Attach the .pdf to the patient file in your EMR. I have not yet tried to

integrate specific fields into the patient file, eg: " I have difficulty kneeling

4/5 " - and to that extent I still have " double entry " .

Once the data is in the EMR I can use it to set goals, eg: " Improve kneeling

from 4/5 to 2/5 " .

For Progress Notes you just have to repeat the process - make data collection

part of your clinical routine so staff don't feel any additional burden.

Tim,

How do you get the addresses to contact the authors? Most of these

tools are handed out at various seminars without instructions to contact

authors for permission to use the tool.

You can usually get their e-mail from the full text version of the original

article.

Many publications (eg: Spine, Arch Phys Med Rehabil, etc.) DO NOT provide free,

full text copies even though their research is publicly funded by the American

taxpayers.

This site claims that the ODI is public domain for clinical use:

Most times a Google search will find questionnaires residing on private servers

for patient use...

You should be able to spell out the questionnaire name and find them.

Diane,

If I remember correctly, the DASH was the only tool listed below that requires

registration. They may have changed their stance, but you can find it here:

The main two software options that I am aware of for capturing outcome data are

either FOTO http://www.fotoinc.com/

or another model based on research outside of the United States - AdviseRehab:
