We also getthe intensity for those who are discharging on Monday or Tues. Staffing is the biggest difficulty, as I am sure customer service would improveif we could cover all seven days the same. If our census is lower, we provide an hour of each discipline, with the priority going to patients being discharged early in the upcoming week and for those who need family training when the family has difficulty coming during the weekdays. You can't provide 5 out of 7 days of therapy without working Saturday. We see patients who require 90 minutes of therapy on Saturday and provide others at least 30-45 minutes of therapy. The amount of time and discipline depends on the staffing we have after we have met the 3 hour requirements + our RUG groups on skilled and our acute orthopedic population. Ideally, we like to provide one extra 3 hour day as a buffer for those weekdays when unexpected things might occur that causes us to lose a day. We always found that regardless of the day of the week, (and as long as we at least me the minimum requirements), it was in the best interest of the patient and the facility to provide the patient with exactly what they needed to get home (or to the next level of care) as safely and as quickly as possible. Now this could create scheduling challenges, but good creative use of personnel and generally being open to flexible schedules of staff allowed this to happen. Good Luck! That session length varies depending on where they are on hours of therapy through the rolling week. Depending on staffing and pt need we can increase the time to assist with meeting the regs. We try to PT and Speech on Saturday and OT on Sundays so that they receive some type of therapy session 7 days a week.
