I have found mine to be more problematic recently too. I have been running 150/100 recently despite Cozaar, Lasix and Lopressor. I think unfortunately that it does tend to get harder to manage as you lose renal sufficiency. You are not in this boat alone! Let us know what your doctor says on the 13th. I'm hoping that will change if I can ever get a tranplant. I had much the same problem as you with the blood pressure - pretty much from around 1990 to almost the time I started dialysis. You don't even have to have kidney disease. It seems to be in the nature of blood pressure medications that for some, they work for a while and then gradually lose their effectiveness. Me, well, no amount of any combination of meds worked at all unless one was a diuretic. This is one I would completely defer to Pierre and others with more experience than I.... however, from our limited (1.5 years) experience, it seems to be a matter of perpetual vigilance and perpetual tweaking. 's current combo is 15 mg lisinopril in the AM and PM, along with 10 mg Norvasc in the AM and another 5 mg in the PM (a bit much for a 13 year old!). We are currently on our third episode of trying to stay under control. It seems that we spend 2-3 months getting meds adjusted, after which everything is find for another 2-3 months ---- and then we start the whole cycle over again! Lest I seem alarmist, I should mention, that BP control for MPGN tends to be more of an issue than for IgAN folks. However, it is a very serious issue for everyone! When it senses that more filtering is needed in the kidneys, it uses them. Any drug you use, it can get around somehow. That's why many of us end up taking an ACE inhibitor or an angiotensin II receptor blocker (to partially block the renin-angiotensin system which the kidneys themselves can control by simply secreting more renin), a calcium channel blocker (to directly dilate the smooth muscle of the blood vessels), perhaps a beta blocker to mainly slow down the heart rate and the pumping capacity of the heart, a diuretic to remove more sodium and fluid, and if all that doesn't work, the dreaded clonidine is trotted out. That one works mainly in the brain. There are a number of lesser-used drugs also. The minute you try to lower BP artificially, the body's systems for raising BP get into high gear - but it does take them a little while to catch up. If you block one, another takes over. In the end, nothing succeeds like taking the sodium out or not putting it in (not that I'm suggesting anyone diet without being advised by their doctor). Guest guest Posted January 4, 2004 Report Share Posted January 4, 2004 Drugs like Diovan are thought to help lower proteinuria because, aside from lowering the body's overall BP, they specifically lower intraglomerular BP - lower BP in the glomeruli, less protein is pushed through the glomerular walls. That's the theory, anyway. There may also be some healing properties on the blood vessels that we don't yet understand, and the glomeruli are just clumps of very tiny blood vessels. Because they lower the intraglomerular BP, another consequence is that these drugs actually lower your kidney function a little, but that's of no significance. I don't quite understand it all, but the Diovan is supposed to help lessen the amount of protein spilled. Anyway, Hang in there, but really be aggressive on treatment. I'm on 80 mg Diovan and salmon fish oil pills, so I'm always afraid my neph will need to give me more aggresive treatments!
