There are now agents (either currently approved or investigational) that not only reduce blood glucose but also: 1. Work through the glucagon-like peptide (GLP)-1 pathway 2. Result in increased beta-cell mass 3. Directly target the pathways associated with programmed cell death 4. All of the above ============================= We know from serial observation studies that a decrease in beta-cell mass and a reduction in beta-cell function, which can be detected in autopsy studies, is characterized by an increase in beta-cell death. This is important because there are now agents that not only reduce blood glucose but also directly target the pathways that underlie this beta-cell death. One of the new categories of agents are the GLPs, exemplified by exenatide. GLP-1 can directly stimulate cell proliferation, directly inhibiting beta-cell death.
