She has had longstanding depression and has treated it in the past successfully with Zoloft. She was placed on Zoloft prior to delivery, 25 mg, and feels she needs more than this. I concur and so she was given a refill of 50 mg to be followed through with her PCP. The second issue is that she has a chronic UTI and that was recently evaluated this month, as a matter of fact, with Dr. M, and he is expecting to need to see her in a year. The third issue is birth control. The patient has not resumed birth control pills. She does need to consider that, and I have encouraged her to follow through on her appointment and reevaluate this within the next month. A: Depression, chronic and XXX(s/l...vaginous).
