The first one is: " She had an accident on s/l 'benzovasopine' overdose and currently is taking Paxil 20 mg q.d., amitriptyline 25 q.h.s., Ativan 0.5 p.r.n., and Xanax 0.5 p.r.n. " The second one is: " Sixty-year-old in today. Needs update immunizations. Going to India. Had complete physical first of the year. Ate breakfast at 8:30 this morning, five hours before admission here. Needs refills on his Lipitor, Diovan, and Lidex cream, also malaria tablets.
