Guest guest Posted August 31, 2001 Report Share Posted August 31, 2001 I'm going to post the entire report to give you the whole picture..(if anyone sees anything in this report that just is not correct or typed wrong or whatever, please don't hesitate to let me know.. XXX presents today because her #7 uncuffed fenestrated Sims Portex tube came out. She was seen in an emergency room at which time they placed a #4 Shiley which has not provided her with an adequate airway. The #4 Shiley was removed. The peristomal skin was cleaned with Hibiclens. The granulation tissue around the edge of the tracheostomy was cauterized with silver nitrate. A new #7 uncuffed fenestrated Sims Portex tube was then inserted without difficulty and her airway was excellent. A second tube was given to her in case she developed any further problems, and the s/l (salad or satellite) for the tube that was placed in was also given to her. Careful instructions were given as to the different parts and what would be needed if the tube needed to be replaced again. Hopefully, we will not run into this problem again.
