Aliceanne, Left ray resection would be correct, I hear this quite often.

PS Please, please, don't put things in bold and underline and expect us all to

see it. Probably people who are using the same ISP will be able to see that,

but for those of us using a different ISP, everything in the report looks

exactly the same. Please use the s/l or quotation marks or something else to

set off the words you want us to check. I hate having to read and reread the

whole report trying to find the word/words in question. Thanks, Margaret

Doctor dictates the following. I am questioning in the bold, underline

section the words 1st RAY resection. Or is she maybe saying re resection.

As you can see she uses abbreviations nauseatingly.(word?)

Pt. seen 74-days S/P surgery, Lt. foot. Pt. also C/O long thick nails,

B/L.

Exam: the Lt. foot is seen with no further evidence of wound opening.

There is normal healing with no erythema, edema, or ecchymosis

surrounding the healed area. All nails are elongated, hypertrophic,

yellow, dystrophic with crumbly S/U debris with dark discolorations.

A: Normal wound healing, 74-days S/P left 1st ray resection.

Onychomycosis with DMPVD.

P: 1. Aseptic onychoreduction X 10 OM nails.

2. D/C use of Regranex and daily bandaging Lt. foot.

3. Dispense Lazer cream with Vitamin E to be applied to Lt foot area of

cicatrix.

Advise pt. to purchase shoes with proper width, length and depth and

bring into the office prior to wearing.

R/A: One week or PRN.

Thanks. and Sorry. I did not realize that the bold would not be the same

or the underline. I figured other features would be different. Learn

something new every day. Thanks again.
