I'm sorry. There are a couple of other angles you can pursue. Is

your employer self-insured? If so, your HR dept can help you. Or

you can call your state insurance commissioner's office too.

Sometimes a simple phone call from them to a ins. co. scares them

enough to approve a claim. You can print out the AAFP and AAP

guidelines on treating plagio which mention banding.
