Please type in Pain Journal in an internet search and you will find several options. If my pain is not being taken care of, I put in my medications, when I take them, when they wear off, and this way my Doctor can see if it is the time of the day, the type of medication, or what the pain levels are when I took it and what pain level remains when I took it. Pain medication to be effective should be reduced to under five. This is the pain level they strive to maintain as functional -Functional activity: I put down what I can and cannot do to let my doctor know that the program we are doing is allowing me to function at. These are called daily activity, I put in I lie in bed 8 hours a day as I cannot get up, when I cannot. I put I can dress myself etch but cannot drive myself to appts, lift, do gardening, shopping. -Triggers: I put in that I did garden work for two days and caused be to over exert myself, thus a trigger to make the pain med unable to control my muscle spasm. Weather, Stress, and other information goes here. -Other infor: I put in my stretches, heat, topical ointments and other things I do. This allows me to show my Doctor what I am doing to assist so he can adjust my program. Yours will be different but these are common items. I will be happy to send you a copy of mine if you would like to your home e-mail and the links I gave give you ones already made up, you just type it in. Bennie Here are the links I posted and the worksheet would help you but if it is frustrating concept to you, and do not need it, don't use it. No score, just documentation of how you are doing on your current pain plan and it allows the doctor to have information about you to change it in the areas you need it changed.
