Sorry for the empty response before. Lets try this again.

If you go to Files and then to Insurance help you will find a wealth

of information to help you with your appeal. Does your child have

torticollis (sorry, I can't remember)? I had previouly read a court

case in North Carolina that stated that the childs Plagiocephaly was

directly related to her congenital torticollis and therefore is was

a medical condition, not a cosmetic one. Sorry, I can't remember

all the details, but you can view it at www.harp.org/bynum.htm.

I'm sure some members experienced in this will be able to provide

you with added assistance.

I just send you to your personal e-mail my appeal letter.Only because I don't want to bother everybody else with an attachment but if somebody need it I can forward it.

It contains tons of very useful information

Also is right, in the files you can have a lot of information that will help you.

I'm so late in reading this......thank you for replying. It's just

basic Plagio....nothing else. I'm so overwhelmed...I'm hoping to go

through all of Ivonne's links and appeal stuff. I think my case may

be alittle different than her's though. Thank you, any other links

would be greatly appreciated:)

Hi :

I enclosed every document in hard copy although I put the URL at the end (in the cases I got the information form the internet).

In the particular case you are mentions, yes, I included a hard copy, I just seach it in the internet and I pinted it out.

I also enclosed:

Letter of Medical necesity from my doctor (plastic and craniofacial Surgery)

United health care denial letter

The plagiocephaly evaluaction from Children's Medical center od Dallas

FDA (food & drugs administration) report that shows that Cranial technologie has listed this device and registered their facility in accord with DFA regulations (this document was provided by cranial technologies).

The estimate from cranial technologie showing the cost of the device ($3000)

Copy of my insurance policy (provided by my human resource dept because UHC didn't want to provide it), showing that portion "What IT is covered under my plan"(Durable medical equipment) & what IT IS NOT covered under my plan.

Let us know how it goes. I'm still waitting for their responmse. I sent my appeal letter back on Sept 24 and they told me that it will tood 30 days. Let see...
