I may be mistaken, but I think the policy used to state that a cranial vault asymmetry of 6mm or more would indicate treatment would be covered (perhaps moderate?), but I see now that it only mentions that a physician must diagnose the baby as moderate or severe in asymmetry. Further down the page is a cephalic index matrix devided by gender, age, and standard deviations. Aetna Clinical Policy Bulletin #0379: Cranial Remolding Band/Helmet http://www.aetna.com/cpb/data/CPBA0379.html Having no formal, industry-wide assessment of what is mild, moderate, and severe doesn't help with consistency of recommendation of helmeting or banding, unfortunately. I believe a standardization of assessment of measurement is in development in the orthotics industry, so perhaps this standard will be adopted in the future among the specialists, too. They in know way explain measurements. I think I will investigate further where I can find such info.
