Fee schedule for cash paying patients

Has anyone set up a fee schedule for cash paying patients?

If so--have there been any concerns with:

1. Medicare? If your fee schedule is not LESS then what the Medicare

fee schedule is, has there been any red flags?

2. Medicare? Do you have them sign an ABN?

3. Do you even bill Medicare--or just have them pay the cash based fee

schedule?
