CHARGE Communication Survey

The University of Tennessee CHARGE Research Group is conducting

an online parent survey on " Communication Development in CHARGE. " The

results will be compiled and shared in a booklet and a poster at the Conference

in Miami Beach. If you would like participate, please access the invitation and

survey at the CHARGE yahoo groups web site at

http://groups.yahoo.com/group/CHARGE/files . You will require your yahoo ID and

password to gain access to the files section.

Question: Will we eventually be given the website to log in our survey

responses, or should we type in our answers in the word document and email

it, or print the survey out and mail the responses? I apologize if the

answer to this question has already been posted....

We are conducting this survey to gain additional information about these

factors from parents of children with CHARGE about these factors. The

survey will be conducted online at a website. ......>snip<....... A

compact version of the survey accompanies this letter. It is a preview with

all of the questions you will be asked. Please read through it before

deciding to participate. We estimate that it will take about an hour to

complete - but we request that you look through your child's records and

assemble the information you need before you begin.

If you wish to participate, please go to this website [XXXXXXX] and complete

the survey online. Participation is completely voluntary and anonymous. We

will not be able to identify you or your child. If you have to stop halfway

through, there is no way to restart where you left off. So we request that

you have the information about your child close at hand when you begin.

Oops! The wrong file was uploaded. It was not the final version which

has the website in it. I'll contact Jim Thelin and ask him to send

the right one. Also all parents who are members of the Foundation will

be receiving hard copy of the information in the two files very soon if

you want to wait.
