Please keep in mind that effective April 28, 2008, CMS

will no longer accept signature stamps on

documentation that could be requested and reviewed by

Medical Reviewers at the Medicare contractors. Here is

the link to the Transmittal. Even though it states

hospice services at the beginning, if you read

further, it does apply to all services and this was

confirmed with CMS.

Rick,

When you say this was confirmed with CMS, who confirmed it with them? Was

it the HPA section of APTA? Just wondering as I spoke to our Corporate

Compliance Department recently and they are still trying to figure out who

this applies to. Thanks!

Rick,

So to confirm, this now applies to Medicare Plan of Care signatures? I

thought they got rid of the " no stamped signature " rule a few years ago. Is

this, in essence, rescinding that?

What is the reference again for this?

How about electronic medical records? These usually have a signature that is

digital. I have seen some clinics that even have a paper note that is scanned

into the computer, then the paper note is shredded.

Rick,

The link you provided relates to signature requirements for the

certification of terminal illness for hospice patients (Transmittal

248).

Is there another recent transmittal that specifies that signatures must

be hand signed or electronically signed vs. stamped that relates to

non-terminally ill Medicare patients?
