Returning patients

Question, pt. presents to a private outpatient PT clinic with an

initial referral for ft. pain. Receives a couple weeks of care and

disappears. Returns one month later (pt. had been hospitalized for

other medical issues) with a new referral for UE pain. A D/C had not

been completed. How do you proceed with documentation and billing? D/

C pt. which should be done two weeks following the pt's

absence,automatically? Re-eval the ft pain? Initial eval. the UE

pain? Should the pt. schedule separate appointments because they have

two separate problems? Thank you for your feedback.
