I just went through the process, it took close to 180 days to get my

medicare number. Make sure you proof read every section of all forms, in order

to

aviod any mistakes.You will be able to provide services to medicare patients,

but are not allowed to bill for these services until you or your group are

approved.Mail everything together by return receipt . Keep a record of all phone

coversations. Best time to call CMS is on mondays , at 8a.m.

Good Luck is all I have to say ... My husband and I opened a private

practice in Ormond Beach, FL in November 2006 and he waited over 6 months to

even get a response back from Medicare. This was only after we contacted

Mica's office for assistance due to the lack of response we got when we tried to

follow up. We kept getting told that the application was " in process. " The

only saving grace is that you are allowed to see Medicare patients while you're

waiting for the number, you just can't bill until you receive it. Hopefully

you'll be able to get in network with some other commercial insurers to help

your cashflow while you're waiting on Medicare.

Medicare will allow you to retroactively bill. Once you submit your application

you can see the patient but dont expect to get paid until you get your number

and then re-submit the bill. This will likely take 4-6 months.

Thanks Jim, I am reading the forms over and over to make sure I don't make any

mistakes.

Did they send back any forms to be resubmitted or it just took 180 days?

In the state of Texas you have to register your facility and you would put

the registration number in that spot. Hope this helps.

If you are referring to Section 2 (page 8), the answer is " no " . A PT or OT

group practice in Florida is not required to be licensed or certified by the

State of Florida.

As an FYI - FCSO (Florida Medicare) is having an " Ask the Contractor "

teleconference on Part B enrollment on August 16th. Let me know if you

I practice in Florida and have my license to practice here. I was told

this did not pertain to my individual license by a customer service rep

at medicare however, he did not know if a private practice needs its

own state licensing or certification.

Hi ,

In Illinois, I had to get my state licensure from IL dept of professional

regulation and it took them 6 months before we got our professional license.

Check with your lawyer, he should know. In Illinois, since we are a medical

professional office/ business, we had to do this before we can be in

operation.

Hi, -

In Florida, the practice needs to have a local business license. This

may be checked through your City Hall and your County Tax collector's

office. There is not a State license, per. se.

After seeing Dick's response :-) I wanted to add this to my original

statement (PT/OT clinics are NOT subject to clinic licensure by the State of

Florida). Dick was referring to local business taxes (often called

occupational license/tax) requirements.

The following is applicable to Fort Lauderdale (in the city) and Broward

County. Check with each governing authority to ensure you have the correct

forms, and if the applicability is to each individual or to the clinic as an

entity. Within Florida the requirements vary by city and by county.

Fort Lauderdale Business Tax:

" The Business Tax is a tax for the privilege of engaging in or managing any

business, profession or occupation within the city limits. In order to

operate a business based in the City of Fort Lauderdale, you will need to

pay the Business Tax. Whether your business is operated from a commercial

location or is home-based, the Business Tax is required pursuant to both

local and state laws. The business tax receipt is valid from October 1

through September 30 of the following year. Prior to expiration, the City

will mail you a courtesy reminder notice to remit your payment for the next

year. Once your payment is received, your tax receipt will be mailed to you.

When you receive your tax receipt it must be posted in a conspicuous place,

visible to public inspection. "

Broward County Business License:

" Any person doing business in Broward County needs a Broward County Local

Business Tax Receipt. Businesses will also need to obtain a city license

from the municipality that the business is located in. The Local Business

Tax Receipt is a tax imposed for the privilege of doing business in Broward

County. Any person who provides merchandise, entertainment, or services to

the public, even if only a one-person company or home-based business, must

obtain a Local Business Tax Receipt to operate. "
