Make sure you proof read every section of all forms, in order to aviod any mistakes.You will be able to provide services to medicare patients, but are not allowed to bill for these services until you or your group are approved.Mail everything together by return receipt . My husband and I opened a private practice in Ormond Beach, FL in November 2006 and he waited over 6 months to even get a response back from Medicare. This was only after we contacted Mica's office for assistance due to the lack of response we got when we tried to follow up. We kept getting told that the application was " in process. " The only saving grace is that you are allowed to see Medicare patients while you're waiting for the number, you just can't bill until you receive it. Hopefully you'll be able to get in network with some other commercial insurers to help your cashflow while you're waiting on Medicare. If you have any questions, feel free to contact me. Once you submit your application you can see the patient but dont expect to get paid until you get your number and then re-submit the bill. This will likely take 4-6 months. A PT or OT group practice in Florida is not required to be licensed or certified by the State of Florida. Check with your lawyer, he should know. In Illinois, since we are a medical professional office/ business, we had to do this before we can be in operation. Hope this helps. Dick was referring to local business taxes (often called occupational license/tax) requirements. The following is applicable to Fort Lauderdale (in the city) and Broward County. Check with each governing authority to ensure you have the correct forms, and if the applicability is to each individual or to the clinic as an entity. Within Florida the requirements vary by city and by county. Fort Lauderdale Business Tax: " The Business Tax is a tax for the privilege of engaging in or managing any business, profession or occupation within the city limits. In order to operate a business based in the City of Fort Lauderdale, you will need to pay the Business Tax. Whether your business is operated from a commercial location or is home-based, the Business Tax is required pursuant to both local and state laws. The business tax receipt is valid from October 1 through September 30 of the following year. Prior to expiration, the City will mail you a courtesy reminder notice to remit your payment for the next year. Once your payment is received, your tax receipt will be mailed to you. When you receive your tax receipt it must be posted in a conspicuous place, visible to public inspection. " Broward County Business License: " Any person doing business in Broward County needs a Broward County Local Business Tax Receipt. Businesses will also need to obtain a city license from the municipality that the business is located in. The Local Business Tax Receipt is a tax imposed for the privilege of doing business in Broward County. Any person who provides merchandise, entertainment, or services to the public, even if only a one-person company or home-based business, must obtain a Local Business Tax Receipt to operate.
