AM-PAC and other Transmittal 63 questions

Transmittal 63 lists several measurement instruments to document

patient progress. One of those is " AM-PAC " (Activity Measure-Post

Acute Care). I am unfamiliar with this. Can anyone steer me to the

provider of the instrument and/or sample questionaires? Also, anyone

who has used it: can you comment and compare it to the other

instruments? And finally, my read of 220.3 Section C ( " Documentation

required to indicate objective, measureable beneficiary physical

function... " ) is that we MUST use some sort of formal, validated,

measurement instrument in our Part B documentation; is that the true

interpretation of the rule?
