Is a Functional Capacity Evaluation necessary or is that just for Word Hardening? If a FCE is needed, how can I obtain one of these forms? If not, what type of Evaluation tool is used? Also what ICD-9 code is used in the fee schedule? Are there any websites that would be beneficial for me to gain additional insight into offering this type of program? Thanks for your help.
