Does anyone have a reference delineating whether or not and how much

documentation time is included in a patient's treatment session? Is it

an issue of timed v. untimed coding?

Documentation time is never a billable function under Medicare Part A or B.

Only direct resident time counts towards minutes under A or CPT under B. If

a therapist is very efficient he/she may be able to complete some

documentation during the treatment session while explaining to the resident

progress or results of the evaluation, etc.
