Guest guest Posted September 27, 2006 Report Share Posted September 27, 2006 Shari, If these people you are referring to are over 55, you may want to see if there is a PACE (Program for the All-Inclusive Care for the Elderly) in your area. This can be a good solution for those who have family support but need more help than HH can provide. PACE programs provide transport, OT/PT/ST, MD, RN, Activities, at least one meal, family training, DME, etc...everything possible to allow the patient to continue to live intheir own home as long as possible. We have had some patients lately that we recommend they have 24 hour supervision for discharge home due to safety issues or require assist for all transfers. The family and pt are educated on the reasons for the assist but when they arrive to pick up the patient they state that the patient did not want anyone in the home so they do not have the recommended amount of care. Should we cancel the discharge until proper care is in place, are we responsible if they go home then are injured or end up hospitalized shortly after D/C home?
