I had a very good friend who had a kidney transplant and the way she was able to keep her sugars at a good level was because the pump gave her increased insulin amounts at the times when her glucose levels would be elevated. Re: Splitting Lantus dose Is there something besides prednasone that works for you? Steroids of any kind mess up the blood glucose level plus a lot of other things. My granddaughter is on steroids due to a birth defect for the rest of her life. We have to watch her very carefully, and help her avoid infections and other problems because this steroid suppresses her immune system. RE: Splitting Lantus dose , for one who is not yet testing and not familiar with the meters and test results, what does it mean when you say you were: I was 6.8 before dinner, 6.1 after dinner, and then dropped to 4.7 half an hour later, and then was 12.7 by the time I went to bed. Mine is 8.3 controlled with 2 needles per day and extra humalogue if needed. How big are the pumps now? They used to be fairly big so I was turned off by them. Have you ever heard of an Omni Pod? Small! Her A1C ran under 6. She, unfortunately, had astroke, so is no longer with us. She was diabetic for over 50 years. To convert them to mg/dl which is used in the United States, just multiply by 18. So a reading of 6.8 mmol/l is a reading of 122, a reading of 6.1 is a reading of 110, a reading of 4.7 is a reading of 85, and a reading of 12.7 is a reading of 229, and so on. To convert backwards (from mg/dl to mmol/l) just divide by 18. Reading in mmol/l are not the same as A1c readings, in that a glucose reading of 6.8 average does not mean you have an A1c of 0.068 or 6.8%. When I discussed switching from NPH to Lantus with my Endocrinologist, I came away with the impression that the main advantage of Lantus was that you only have to take one shot in a 24 hour period. My doctor also left me with the impression that Lantus would provide a relatively uniform absorption of the Lantus over the 24 hour period. Lantus has a 20 hour absorption life in your system. You are splitting your Lantus dosage between two shots to cover the four hour shortfall. You are also having problems juggling the eight hour overlap when you split your daily Lantus dosage into two shots. I don't see the advantage to you to take Lantus over NPH if you still have to take two shots a day. Does your doctor think that Lantus still has an advantage over NPH in this situation? Please do not take my question as a criticism of either your decision to take Lantus or to stay on Lantus. I am asking the question for information on my own situation. I still have not decided to switch to Lantus yet. Guest guest Posted October 15, 2006 Report Share Posted October 15, 2006 The main advantage for me for Lantus is that it had virtually no peak. I do not need to snack between meals and if I eat a meal an hour or two later than usual, I do not go low as would happen on NPH. As a college student with a vary variable schedule it was almost impossible to gain any control with NPH. I do not yet know whether splitting Lantus will work. I only split the dose two days ago and so will give it a few more days before deciding if it needs more adjusting. I did a 50/50 split taking 10 units in the morning and 11 units at night. Many people I have talked to actually do a 40/60 or even 20/80 split of their Lantus and find that this works better, so this may be what I end up doing. I also did not adjust any of my Humalog doses when I split the Lantus, even though I was likely using dinner's Humalog to cover some of the gap in the evening, so this may also be what is causing me to go low and I will adjust tonight. When you split a Lantus does it does not really overlap, not in the sense that you are getting more Lantus at one time than another, because you do not double your dose but simply split it between two shots, so when those two shots overlap it creates the same dose as you had before, just with a longer coverage. Most people start out with one dose of Lantus and only split it if they have problems, so for many people it is a once-a-day insulin and does work for the full 24 hours. I am actually hoping to go on the pump as soon as I find out whether insurance covers the cost of buying one, and if this happens then we will be able to fix most of my problems more easily.
