As far as what happened to you, there is some wrong info here. Nobody is " an

ultra-slow metabolizer for CP450 drugs " . You can be an ultra-slow metabolizer

(USM) for a couple of P-450 isozymes, but you would never be an USM for the

entire P450 system isoform. In fact, you are likely to be an extensive

metabolizer of at least one of these isozymes.

Doxepin is primarily metabolized by CYP2D6. CYP1A2 & CYP3A4 as minor

pathways. 7-10% of Caucasians have reduced activity of CYP2D6 rendering them

poor metabolizers of Doxepin (and certain other psychotropic drugs).

This generally wouldn't be a big deal given the low dose. But I would need a

thorough history to go any further. In an atypical situation like this, it

requires me to be as thorough as I can be because it's like solving a mystery. I

can only offer you solutions once we are sure about the root of your problem.

I work at this intensive level with people but not on the list as it is too

tedious and takes too long to do in this setting. If you're interested in

pursuing this, please let me know.
