Is each session billed independently (ie... am session 25 minutes 2 units and pm session 26 minutes 2 units) or are the sessions combined for total minutes (51 minutes 3 units)? There is some controversy among our staff and I am looking for clarification and any resources on current Medicare billing policies that might be helpful. Also, do you document two separate notes for each session or document daily with your treatment times for each session on your daily note? I would appreciate any information anyone has on this topic.
