CMS-1500 Billing Question

Hi,

We are a new clinic in Northern California and are having problems getting

reimbursed for PT from Medicare. They have denied our claims for the past 3

months.

I was told 2 different things from Medicare about entering data in the CMS1500.

1. The dates on Box 14 (date of illness) and box 19 (reserved for local use)

should match (last seen by doctor). Box 19 needs to have the doctors U-PIN

number

2. Box 19 should have the date last seen by the doctor and the doctors U-PIN

number where as Box 14 should have the date of first symptoms.

Can you please let me know which one is correct or if both are wrong ?
