Acute Care Nursing Initial H/P Rehab Triggers

Our OT acute- med/surgical utilization is decreasing.

We do not seem to be getting the trigger for OT evaluations on the

acute-med/surgical nursing unit initial H and P intake form.

I am requesting examples of effective OT (and PT) nursing screen trigger

criteria to improve utilization.

Any examples would be greatly appreciated.
