We have been told by our FI that Vital-stim provided without any other

swallowing therapy techniques integrated into the therapy session is

non-reimbursable. Vital-stim per Medicare and ASHA is still considered

experimental. If Vital stim is being used in conjunction with other swallowing

therapy techniques then the 92526 code can be used. Speech can not use Ther-ex,

E-stim or Neuro Re-ed. If you need this confirmed I would have your Speech Path

contact ASHA or you may contact your FI.

I have had recent discussions with Mark Kander at ASHA. ASHA does not say

that Vital Stim is experimental althugh there are a number of efficacy

studies going on. Has anyone looked at the outcomes and efficacy of thermal

stim? Give me a break and do not perpetuate this misinformation. The only

thing to understand about Vital Stim re payment is that it cannot be coded

using an e-stim CPT. It is totally appropriate to code it under the general

swallowing treatment code and therapists should continue to advocate with

any FI who denies treatment.

neuromuscular re-ed (when used with a 59 modifier) as viable codes for slp

treatment. Take a look at the most recent component codes listed under the

swallow code in the CMS CCI edits. Having said that, SLPs must document the

medical necessity and clearly differentiate the differences when using these

codes.

Just reporting what our FI told us last week - they are not happy with the

studies and therefore are interpreting the Medicare regulations in their own

way. At the moment they are not paying for it.

To my knowledge ASHA has not retracted the information that was published a few

months ago in the ASHA Leader concerning Vital Stim. It was quite clear from

the information provided that ASHA's stand on the issue is that without valid

clinical studies Vital Stim is an experimental treatment that requires

additional investigation and research. This has been repeated to me by my FI. My

FI has instructed me as I stated earlier on how to be reimbursed for swallowing

therapy incorporating Vital Stim. I'm not perpetuating any myth. I'm just

stating the information that I know to be factual from my FI. " G " If you will go

to the ASHA web-site and download the 2005 SLP Medicare Fee schedule on page 6

it clearly explains that despite arguments presented by ASHA, CMS continues to

prohibit SLP's from reporting PM & R codes 97110 Ther Ex and 97112 Neuro Re-ed. If

you want to use a 59 modifier and get paid for them go ahead. If you've been

instructed by your FI to bill that way great... but I'm going to follow the

instructions I've been given directly by my FI. Also FI's don't deny treatments

they deny payment. As far as Vital Stim goes I think its one of the best

modalities that has been developed for Dysphagia patients in decades. My staff

has had great results with most patients. But until opinions on its efficacy

change I'll be billing according to my FI.

I have had recent discussions with Mark Kander at ASHA. ASHA does not say

that Vital Stim is experimental althugh there are a number of efficacy

studies going on. Has anyone looked at the outcomes and efficacy of thermal

stim? Give me a break and do not perpetuate this misinformation. The only

thing to understand about Vital Stim re payment is that it cannot be coded

using an e-stim CPT. It is totally appropriate to code it under the general

swallowing treatment code and therapists should continue to advocate with

any FI who denies treatment.

Becca,

UGS reimburses for 92526 whether VitalStim is provided

as a component of the normal dysphagia treatment or

not. They will not reimburse for G0283 on the same day

as 92526 as there is a CCI edit that prevents this. If

you would like to see this in writing, please e-mail.

They will be posting this soon to their website under

PCOM FAQ's.

We do not bill for e-stim or therapeutic activity. We incorporate VitalStim

into our swallowing treatment sessions and bill for traditional swallowing

(dysphagia) therapy only (CPT 92526).
