bicornuate/septate surgery question (pg mentioned)

Hi everyone...I posted last week about having a surgical consult

appointment regarding septate versus bicornuate uterus. As a

background, I gave birth last July to a healthy 33 weeker and just

had an HSG on the advice of my OB. The radiologist said I had

a " complete septate uterus " . Today I saw a new doc, Chair of the OB

department at a great university hospital (and also a fertility

specialist), and he said there " is no way to diagnose a SU from a BU

with an HSG. " Hello? Is this true? Of course, the bunny ears are

quite visible on the HSG, but I guess that's just the cavity and not

the shape of the uterus. That was news to me. He recommended that

I have an MRI done - is this what others have had? He also said

that he could perform surgery on both a septate and a bicornuate.

I've read through all the postings on the site and it seems there

are a lot of people with resected SU, but I haven't heard anything

about surgery on a BU. Have I missed it? The BU surgery would be

via a c-section.

Thanks for any advice you can give. As you can tell, I'm a bit

confused by the fact that neither my OB nor the radiologist knew

that an HSG was the wrong test. Unless, it wasn't.....

Very interesting, Mindy! I too am having surgery performed by the

Chair of the OB department at a great University Hospital (and also a

fertility specialist) (but as I'm in Australia I doubt it's the same

surgeon! My hospital is the Royal Hospital for Women, Randwick (in

Sydney), which is attached to the University of New South Wales).

Anyway, Prof said he too could repair a bicornuate uterus, but

he wouldn't do it on me if mine proved to be BU, as I've had two

children, and 2 c-sections. I guess it has something to do with my

uterus not being as " taut " as it was pre-kids?

If you've had one child already, maybe you should ask him if he'd still

recommend the reconstructive surgery on a BU...

Just by the by, I e-mailed Prof and asked if he would use the

balloon thing that others have had after their metroplasty, and he said

no. It's kind of nice to have a doc who's au feit with modern

technology such as e-mail! He writes straight back as well, which is

great, of course, I know his surgery day is Monday and he has a clinic

on Tuesday afternoons, so I don't e-mail him then...he also mentioned

that he's been doing metroplasty for more than 20 years, which is a bit

of a relief to me!

Hi Mindy. The Chair is right. HSG can help in diagnosing but does not give an

accurate reading. I had an HSG then a MRI. The MRI diagnosed me with a Septate

uterus, but the HSG a BU. Good thing the DR didn't go along with the

radiologist. Some people go ahead and have a lap/hyst done to see what the

uterus really looks like. My dr. wanted to do a MRI first to (so did I). My

opinion is that is a good road to take. Many of us here have been misdiagnosed

as BU when we are actually SU. I had the surgery to remove the septum in April

and have a follow up MRI next Friday.

I know this can be very confusing. I still don't understand some of it. I hope

you can get your questions answered soon! Waiting is the worst, but we are here

for you.

Take care,

Hi Mindy. That's true - that you can't diagnose an SU from a BU with

an HSG. I had an HSG which showed a unicornuate uterus. The RE

followed up with an MRI which diagnosed my BU.

I haven't heard of surgery for a BU being recommended....I've heard

it can be done but don't know of anyone who has had it.

The HSG wasn't the wrong test - it's often the test they start with.

Then, if there's a need - they go on to use other tests to confirm

the diagnosis.

Hope this helps.

Good luck.

Hi, Mindy. Your story sounds similar to mine. I gave birth at 30

weeks due to a placenta abruption. I had a c/sect so during the

surgery they got a good look at the outside of my uterus which was

described as " perfectly heart shaped " . A hsg a year later revealed

the same bunny ears sounds like you saw.

I'm wondering why you gave birth at 33 weeks? Preterm labor? IC?

Glad to heard you daughter is doing well.

Thanks everyone for your insight and explanation about HSG versus

MRI for diagnosing BU/SU.

Stacey asked who my doctor is: he's the chair of Obstetrics at

town University in Washington, DC (Craig Winkel). He knows I

have a child, and still thought BU surgery is an option. Is BU

surgery uncommon because it's ineffective or because a heart-shaped

uterus doesn't lead to miscarriages? I'm sorry these questions are

so basic - I'm new to this field!

I'm scheduled for an MRI next Thursday.

- your story does sound familiar! I had my daughter at 33

weeks because my water broke (she ran out of room?). She was breech.

Again, thanks for all your responses. It's nice to have such a

knowledgable and caring group on my side!

Hi Mindy. Looks like Sara gave you great information about why

surgery isn't often done on a BU. In fact, it helped me understand a

lot more about my BU! Thanks Sara.

I think with that knowledge, and the fact that you have a child, I

would resist the idea of surgery if you do have a BU.

Please let us know what they say at your MRI.

Best of luck to you.
