Guest guest Posted May 14, 2012 Report Share Posted May 14, 2012 I'm curious what everyone uses to confirm/bill for a diagnosis of Malnutrition and the different levels-mild, moderate, severe. I currently use prealbumins and BMI's and we calculate the Total Lymphocyte Count as well. What is everyone else using? Guest guest Posted May 14, 2012 Report Share Posted May 14, 2012 Criteria were just published in the Journal of the Academy of Nutrition and Dietetics. They're also available through ASPEN. It is no longer standard of practice to diagnose malnutrition using prelbumin, or TLC. BMI must be evaluated in the context of the person's overall health and wellness goals. It behooves the RD to claim ownership of evaluation of intake. No one else in healthcare has the education and training that we have in determining if someone's nutrient intake is meeting needs. That, and probably some evaluation of weight status, overall functional capacity, body habitus (muscle wasting, fat loss, etc) should be sufficient to make some decisions. Also, look at the research using the Subjective Global Assessment, which is a validated tool for assessment of nutritional status.
