I do not see this type of pt. but I can share the following:

 

I have a Type 1 DM,brother who runs marathons, participates in triathlons and he

definitely has a good grasp on his management.

 

He got into the habit of checking his blood sugar as much as possible before,

during and after given amounts of cycling, running, etc to track his response.

Takes a lot of effort on the patient's part initially but it is one way

of being in control and not ending up with dangerous & scary hypoglycemic

episodes. He carries Goo/protein bars/Gatorade with him at all times. I think he

has even used Extend Bars and found them helpful.

Does anyone have general recommendations for a type 1 diabetic running a half

marathon this weekend? He ran 8 miles a few days ago and had to stop multiple

times with the feeling everything was spinning. He stopped twice to get some

Gatorade and then a bag of chips. When he got home, he was shivering and his

blood sugar was very low. He took insulin 2 hours before he ran.

My thought would be to make sure he includes protein in a snack, such as a

protein bar. Just consuming high carb snacks and then continuing to run doesn't

surprise me that he crashed. What about his insulin? He is thinking of taking

1/4 of his usual dose.

I know it may be difficult to give specific recommendations, but any general

ideas would be much appreciated.

Thanks for your input, .  I am going to refer him to speak with an RD/CDE

who has expertise in this area.  My previous experience working in outpatient

diabetes education was mainly with type 2 diabetics, and I have never worked

with athletes.  I had no intention of even touching the subject about insulin -

I can't make those recommendations.  I do think he should at least contact his

endocrinologist to discuss before running.  I was mainly looking for

suggestions related to what he can eat before, during, and after he runs (ex.

bars).  Again, I am just going to pass this on.  This is beyond my area of

expertise.  I do appreciate your response.  Have a great day!

sport " bySheri Colberg. There is a chapter on endurance sports with

general diet and insulin change suggestions. Marathons are addressed as

well.
