ESTIMATING ENERGY NEEDS

We are updating our use of predictive formulas to estimate energy needs and have

decided to use the Mifflin-St Jeor for acutely ill patients and the Ireton-

for ventilated pts.  According to the ADA recommendations we should use the 1992

version of the Ireton- but I notice that there is also an updated 2002

version.  Any thoughts/comments on which version of the Ireton- is best

would be very much appreciated.

You are doing what we have been doing for the adults in our hospital, based on

the ADA Evidence Library.

 

Although we have started using the Penn State for vented patients (better

correlation than either IJ). With access to the nutritioncaremanual.org there is

a calculator that does all the math for us.  For the IJ, the 1992 equation had

better evidence (per ASPEN guidelines according to our nutrition support RD).

 

I've set up the USAF templates to use MStJ for estimating energy needs.  They

were previously using the kcal/kg but there is NO evidence for that to be found

anywhere.
