She asked me if I need a increase in my inslen and I told her no that I wanted to get on lantis with this humalog that I'm already on. She asked me why I think I need lantis and i told her about my readings 2 hours after eating a meal. I told her that it went up and the insalen didn't take it down any. She told me that was normal. She also reminded me that I was taking Humalog mix 75/25. She said that I'm getting 75 percent of short acting and 25 percent of long acting. So when I told you I was taking just humalog, I was taking a mix. She was proud of me for getting my sugar down less then it use to be. I told her that I'm on a diabetic mailing list and I have a lot of wonderful people on there that's very helpful. Then she went on telling me that my sugar should be between 90 and 130 before meals and my sugars should be less then 160 2 hours after a meal. I told her that it probably would be better if I took 2 different types of insalen in sted of the mix. I probably would get better results with the 2 different types of insalen. My diabetes educator will be back in tomorrow so I'll get to talk to her. I also called my doctor to see if he could do it for me but he haven't called me back yet. I'll let you no what he says. This way, you can adjust both independent of one another to get you in range. The 70/30 mix you ar on... there is no way of adjusting the insulin's independent of one another. If you ran a 2 hour post meal of 160, your a1c would be around 6.5 and possibly higher. The two hour post meal test of 120 or less *guarantees* that your a1c will be between 5.0 and 5.3. Most educators or docs settle for the average of an a1c of 6.0 to 6.5 but Otis, you are not average... you are above average and are going to get your a1c to 5.0 to 5.3! Partner get that humalog and lantus and we'll get you all fixed up no worries. I actually got Harry on the two last year or so and helped him adjust it. I have been doing this for 30 years so know my stuff. When you talk to your doctor or diabetic educator, talk like you been doing this for years and teach it. Be confident. Explain that you understand the following: 1. I will limit each meal to 30 grams of carbs which will require a low dose of Humalog 2. as a type 2 diabetic, I will probably have to dose 5 to 7 units of humalog per 15 grams of carbs. 3. I will *always* do a two hour post meal test to see what my sugar is. I understand that humalog peeks in 2 hours past taking it so this way, I am testing two hours past eating and can catch any lows and eat glucose tablets to counter any possible lows. 4. I believe in small numbers, small mistakes, large numbers, large mistakes. If I eat 30 grams, that is small numbers, if I eat 150 grams at a meal, that requires a lot of humalog and can cause me to pass out. I plan to keep my numbers small to master this. Basically just let them know that you are going to be careful but on the same hand, you know what you are doing. But usually, when you are taking a mix like that the larger number is the long acting insulin and the smaller number is the short one. She is entirely wrong about the measurement though. It should definitely be 120 or less 2 hours after meals. There are number of us on this list that do that and have really A1C because of that. That doesn't mean that we all don't have a higher one occasionally! Keep working on it and when yo at the docs's office ask questions; this office doesn't sound like they are doing much educating. Guest guest Posted August 18, 2005 Report Share Posted August 18, 2005 if you are type one then starting out, you will require 1 or 2 units of humalog per 15 grams of carbohydrates. You probably will need more than that but that is typically the starting point at the low end. 1 or 2 units per 15 grams. I've never met a type 1 that only required 1 unit per 15 grams. so, if you eat 30 grams at each meal you would require 4 units to start. I.E 2 units per 15 grams. Make sense? How you know if 4 units is enough is the 2 hour post meal test. If it is 120 or less, bam, you got it. If it is not 120 or less, next time, up it to 5 units and try again until you get the 2 hour post meal 120 or less. Let me know if this makes sense. You probably will need more than that but that is typically the starting point at the low end. 1 or 2 units per 15 grams. I've never met a type 1 that only required 1 unit per 15 grams. so, if you eat 30 grams at each meal you would require 4 units to start. I.E 2 units per 15 grams. Make sense? How you know if 4 units is enough is the 2 hour post meal test. If it is 120 or less, bam, you got it. If it is not 120 or less, next time, up it to 5 units and try again until you get the 2 hour post meal 120 or less. Let me know if this makes sense. I just didn't no that there were so many different types of that kind of insulin. that's why I just said humalog. I didn't remember those numbers at the end of it though. Most long acting insulins have a peek in them like humalog does. humalog peeks in 2 hours past taking it and is out of your body in 4 hours. The peek is what helps keep your sugar down. However, that is not necessary in long acting insulin. Long acting insulin is used to keep your sugars down in between meals and as you sleep. Lantus is a long acting insulin and is good because it has *no* peek. It also lasts up to 24 hours in most people and only requires 1 dose typically at 8 PM. It is the newest long acting insulin available. Most other long acting insulins have a nasty peek and that can make you low sugared and also you have to split dose it meaning take it at 8 AM and 8 PM instead of the lantus only needing one dose that lasts 22 to 24 hours. One thing you might find out is your units per 15 grams at each meal may be different. I.E for breakfast you may need 6 units per 30, lunch 4 units per 30, and still yet at dinner 4 units per 30. Typically most diabetics require more in the morning. It is just the way it is. When I first started out, I had a lot of fun experimenting with different carb foods to see how they would effect my sugars. However, I did keep my meals at 30 grams. As long as you have each meal at 30 grams, you have that constant in place and can master it. He told me that his doctor that takes care of his diabetes is also a type 2 diabetic. His doctor told him that if he kept his blood sugar 2 hours after meals under 160 that that would be fine. I told him what you all have said. I think he thinks that we are crazy. Most people want to think that their doctors know what they are talking about, and want to listen to what they say.
