Parents, Caretakers, and other interested parties,

The Association For Developmentally Disabled Adults and Adolescents (AFDDAA) is

presently conducting a survey to determine the need for an adult community

for individuals with developmental disabilities in our state. We would greatly

appreciate your help in addressing the specific needs that your child has or

will have in the future in regards to this situation by filling out a short

survey. AFDDAA's mission is to create an inclusive community that would create

housing and job opportunities for adults with developmental disabilities.

Please click on the link below in order to complete the survey. Thank you in

advance for your participation. If you have any questions or would like to be

placed on our e-mail list to receive information regarding the progression of
