50% of revenue? What is the normal billing rate for those codes? What would

that translate to for you? What about no-shows? Do you just sit there and

not get paid?

Hi Sharlene,

I work in private practice in a PCP's office. Technically 99211 and 99212 are

very limited/limited office visit and pay very little. I do not know what the

reimbursement is in your area but we use them when the office visit is 15 mins

or less. I can not see you and the Dr seeing a patient in that time. RD's only

visits are supposed to use 98702 and 97803 for MNT. We need to have an NPI and

be contracted with most insurances. Depending on the insurance some require us

to bill under our own NPI and some we bill under the Dr's NPI. It gets confusing

but I would first check into the reimbursement in your area for the codes.

Technically if he is seeing the patient after then you can bill an office visit

99212-99215 and the patient pays a copay as if they are just seeing the doctor.

I am in private practice in my own office. I do not need an NPI number and

have it, is usually good.

I do understand why you and the physician want to bill under his name and

code. Payment is essential. The problem that I see is the absence of

recognition for dietitians. Unless we are billing as dietitians, we often

go unregnized for our expertise and value. Please do not take offense, this

is just my personal perspective.

If all your expenses are covered, 50% of billable fees seems reasonable.

I work in private practice in a PCP's office. Technically 99211 and 99212

are very limited/limited office visit and pay very little. I do not know

what the reimbursement is in your area but we use them when the office visit

is 15 mins or less. I can not see you and the Dr seeing a patient in that

time. RD's only visits are supposed to use 98702 and 97803 for MNT. We need

to have an NPI and be contracted with most insurances. Depending on the

insurance some require us to bill under our own NPI and some we bill under

the Dr's NPI. It gets confusing but I would first check into the

reimbursement in your area for the codes. Technically if he is seeing the

patient after then you can bill an office visit 99212-99215 and the patient

pays a copay as if they are just seeing the doctor. hope this helps

I have a nutrition clinic at my pediatric GI MD's office once a month.  Which

ever doc is in the office at the time will make a face-to-face visit before the

patient leaves; he fills out the billing paperwork (checks for an office visit),

the family pays the normal copay and he dictates based on my hand written notes

on the intake sheet.   We schedule 4 - 30 min visits. Sometimes there are no

shows but the office staff is pretty good about calling the day or two before

for reminders or cancellations.  The office keeps the copay and I get 100% of

what ever the insurance pays. In return, I do calorie-counts for them (about 4-5

per month). The dictation always states that the patient visited with the

dietitian but the billing is under the doctor visit (because the

insurance otherwise would not pay). This has worked out very well for the past 3

1/2 years.  I think we've only been denied payment once.

face. Using the office visit code gives more flexability thats all. LOL....do

not let the doc go to lunch or step outside for some sun....oh my!

Still don't see why an RD should provide MNT under the doc- isn't that the dark

ages? Yes, we work together, but logistically it is more complex. We should be

billing and receiving the revenue. Glad that's what we are doing out in the NW.

I think this is just the crux of the issue for MNT. Lying under the radar and

just getting paid by MD is an easier sell but not for the viability of the

profession long term. It boils down to money and getting paid for our work. No

one is really listening because this issue isn't being handled at the top level

of ADA but within local state associations. I find it hard to believe that other

professions like the medical, dental and chiropractic associations don't work on

insurance issues for their members at the national level. We are always circling

around this issue and depending on who answers-there is always a different

answer. I'm sure there are legal ways that our national organization can work

with carriers on a national level.

Still don't see why an RD should provide MNT under the doc- isn't that the dark

ages? Yes, we work together, but logistically it is more complex. We should be

billing and receiving the revenue. Glad that's what we are doing out in the NW.

I think this is just the crux of the issue for MNT. Lying under the radar and

just getting paid by MD is an easier sell but not for the viability of the

profession long term. It boils down to money and getting paid for our work. No

one is really listening because this issue isn't being handled at the top level

of ADA but within local state associations. I find it hard to believe that other

professions like the medical, dental and chiropractic associations don't work on

insurance issues for their members at the national level. We are always circling

around this issue and depending on who answers-there is always a different

answer. I'm sure there are legal ways that our national organization can work

with carriers on a national level.

Ah, there is the difference.  I am dealing with pediatrics.  For IBD/IBS,

Celiac, elevated LFTs (fatty liver aka Obesity referrals).   For this population

the coverage is very spotty at best.  The only insurance in Las Vegas that is

offering MNT coverage is Teacher's Health Trust (and I am a provider with them)

with a $20-30 copay for the total payment of $75 per visit, 6 visits per year.

the private insurances are " spotty " in their coverage but generally the big

insurances aren't at least in new england. BCBS, Harvard, Tufts (depending on

the plans) and medicare (DM and renal only) all have coverage. I have very few

rejections but yes it does not mean ALL the insurances have coverage but what I

was saying is that if you have not looked into it recently alot has changed just

in the last 2 years for coverage. thanks
