M-spike???

Doc dictates:

Seventy-year-old XXX returns today. Another problem now. Dysuria,

frequency. But the issue primarily related to a rash on her left

buttock and on her left vulva and inner thigh. She has no fever or

chills. The issue with dysuria, urgency is that she dribbles, and it

just feels lousy. My concern also is recent elevated liver enzymes?

Her serum protein differential was within normal limits. Albumin is

a little low. Nothing was high. There is no s/l " m-spike " . AST was

109. ALT was 106.

Is " m-spike " correct? Meaning?

Karla, If you have the Lil Red Notebook, write this down. I was able to find

it, but only because I had some idea where to look. It would be M spike, but is

found in Dorland's under M component (after looking for M or monoclonal under

IgG). The definition in Dorland's is too long for me to write out, but I

believe the ending is what you are looking for. M components are characteristic

of plasma cell dyscrasias. Also, I believe that I would make all the rest

starting with serum protein differential into one sentence, as all of that is

talking about her liver function tests, unless the doctor dictates all of those

periods, in which case give him what he wants. Just my opinion, Margaret
