Moderators comment Joe (1) A brief content analysis of US President's Emergency Plan for AIDS Relief (PREPFAR) Indicators for India programs presents an interesting insight into the PREPFAR Program goals and objectives in India.(PREPFAR 2005). Presenting, program level target indicators need to be commented as they are an important aspect of enhancing quality of service delivery and over all program accountability. Such program indicators also present a tool to analyse the unmet HIV response needs. The indicators are presented in a cluster of 12 program areas. 1) Prevention of Mother-to-Child Transmission (PMTCT), 2) Abstinence and Be Faithful Programs, 3) Medical Transmission/Injection Safety, 4) Medical Transmission/Blood Safety, 5) Palliative Care: Basic health care and support, 6) Palliative Care: TB/HIV, 7) Orphans and Vulnerable Children, 8) HIV/AIDS Treatment/ARV Services, 9) Counseling and Testing, 10) Laboratory Infrastructure, 11) Strategic Information and 12) Other/policy analysis and system strengthening. The word, `faithful' appears 5 times in this document. `Abstinence' 6 times, `treatment' 6 times, `ARV' 1 time and `condom' 1 time. Looking at the number of indicators for each cluster, it appears that the emphasis of PEPFAR in India is on Counseling and Testing (7 indicators) and Strategic Information (7 indicators) Although, half of PEPFAR funding goes toward providing HIV/AIDS treatment, significantly, there is no indicators on `the number of people ARV treatment'through this initiative. One of the key strategies of Prevention of Mother-to-Child Transmission (PMCT) is the provision for ARVs to ante-natal mothers. There was also opportunities to look into the number of people receiving ARV treatment in program areas such as, a) Palliative Care: Basic health care and support, Palliative Care: TB/HIV, c) Orphans and Vulnerable Children and d) HIV/AIDS Treatment/ARV Services. In the area of policy analysis and system strengthening there are indicators on the " Number of individuals trained in HIV-related stigma and discrimination reduction " but there is in no indicators related to HIV/AIDS and human rights. An indicator on `good governance " of HIV response also would have been another desirable indicator. _________________ Summary Program Level Targets Report: Fiscal Year: 2006 1. Number of service outlets providing HIV-related palliative care (excluding TB/HIV) 2. Number of individuals reached with community outreach that promotes HIV/AIDS prevention through other behavior change beyond abstinence and/or being faithful 3. Number of individuals trained to promote HIV/AIDS prevention, prevention through other behavior change beyond abstinence and/or being faithful 6. Palliative Care: TB/HIV 1. Number of service outlets providing clinical prophylaxis and/or treatment for tuberculosis (TB) for HIV-infected individuals (diagnosed or presumed) in a palliative care setting 2. Number of individuals trained to provide clinical prophylaxis and/or treatment for TB to HIV-infected individuals (diagnosed or presumed) according to national or international standards 3. Number of individuals provided with HIV-related palliative care (excluding TB/HIV) 4. Number of individuals trained to provide HIV-related palliative care (including TB/HIV) 7. Orphans and Vulnerable Children 1. Number of OVC served by OVC programs 2. Number of providers/caretakers trained in caring for OVC 3. Number of HIV-infected clients attending HIV care/treatment services that are receiving treatment for TB disease 4. Number of HIV-infected clients given TB preventive therapy 5. Number of individuals trained in counseling and testing according to national or international standards 8. HIV/AIDS Treatment/ARV Services 1. Number of service outlets providing antiretroviral therapy (includes PMTCT+ sites) 9. Counseling and Testing 2. Number of service outlets providing counseling and testing according to national or international standards 3. Number of individuals who received counseling and testing for HIV and received their test results 4. Number of individuals receiving antiretroviral therapy at the end of the reporting period (includes PMTCT+ sites) 5. Total number of health workers trained to deliver ART services, according to national and/or international standards (includes PMTCT+) 6. Number of individuals newly initiating antiretroviral therapy during the reporting period (includes PMTCT+ sites) 7. Number of individuals who ever received antiretroviral therapy by the end of the reporting period (includes PMTCT+ sites) 10. Laboratory Infrastructure 1. Number of tests performed at USG-supported laboratories during the reporting period: 1) HIV testing, 2) TB diagnostics, 3) syphilis testing, and 4) HIV disease monitoring 11. Strategic Information 2. Number of individuals trained in strategic information (includes M & E, surveillance, and/or HMIS) 3. Number of laboratories with capacity to perform 1) HIV tests and 2) CD4 tests and/or lymphocyte tests 4. Number of individuals trained in the provision of lab- related activities 5. Number of local organizations provided with technical assistance for HIV-related institutional capacity building 6. Number of individuals trained in HIV-related policy development 7. However the indicators still appear to be quite satisfactory as most of the important components have been included in the indicators and emphasis has been given to couselling and testing which are one of the most important aspects when it comes to HIV intervention in India.
