I agree that Lupron is probably correct and intact. I suspect influted is probably involuted. Since femurations is not a word, I guess preparations sounds good. Uterine fibroids: Most likely etiology for patient's complaint of chronic pelvic pain. Discussed therapeutic options including hormonal therapy, s/l *bupron* therapy, uterine artery embolization, or surgical therapy such as myomectomy or hysterectomy. Patient is not currently interested in any therapeutic intervention. Follow up p.r.n. 2. Postpartum exam. BREASTS: Soft and nontender with s/l*intact* nipples and no evidence of engorgement. ABDOMEN: Soft and nontender with s/l*influted* uterus. PELVIC EXAM: Perineum appeared well-healed with some residual Vicryl suture with minimal lochia per vagina. 3. Menopausal symptoms: Long discussion regarding the uses of hormonal replacement therapy for perimenopausal symptoms. Patient is undecided about whether she would like to start hormones or not. She was interested in receiving some reading material. Advised, given her extremely minimal menses status post her ablation, that she might get away with starting with a continuous combined regimen although she has an increased risk of having breakthrough bleeding. Discussed oral versus patch s/l*femurations*. Patient given package inserts for Fem HRT and the CombiPatch. FSH was ordered.
