S: CHIEF COMPLAINT: Cervical myelopathy, normal pressure hydrocephalus,

stasis dermatitis. HISTORY: The patient really does not offer any

particular complaints today. REVIEW OF SYSTEMS: He denies any acute

symptoms. He has had no fever or chills. CARDIORESPIRATORY: He denies

cough. GASTROINTESTINAL: His appetite has been good. Bowel function is

satisfactory.

O: Physical Exam: He is alert. Heart: regular. Lung: lung fields are

clear. Abdomen: soft. Extremities: He does have his ___________(s/l

cirkate) boots on, but he has an appointment to get his toenails done this

morning and I do think that the boots need to be rewrapped - they are not on

quite properly.

A: Impression: Overall doing satisfactorily.

P: I reviewed his medication and would not make any changes.

Any ideas?

Bev W.
