Hello everyone! I know I don't post often but I hope those of you

who read daily like I do may have someone in mind that would like to

become a Family Health Partner. Below is the description of the

position. The Position pays $10/hr and depending on the office could

be between 20-40 hours per week. We are very flexible with

scheduling!!

Immediate Opening

Family Health Partner

The Family Resource Coalition, Inc. is looking for the right person

to fill the position of Family Health Partner, for this Children's

Medical Services Office.

The Family Health Partner is to serve as a representative of and

advocate for children and youth with special health care needs and

their families to Children's Medical Services (CMS) and the

community. The Family Health Partner is responsible for maintaining

communication among families, the community, and CMS. The Family

Health Partner (FHP) will provide information on resources to assist

and support families, serve as the contact for families receiving CMS

services, and the area point of contact for educational training

initiatives generated by Family Resource Coalition, Inc.

The Family Health Partner:

Advocates for CMS Families

Assists in creating and sustaining a family centered approach

Maintains a high level of cultural competency

Assists families in crisis with appropriate referral and support

Participates in all Family Resource Coalition, Inc required

activities and conference calls

Participates in CMS Quality Improvement activities

Represents CMS families and Family Resource Coalition, Inc. at

meetings, conferences, and other related activities

Develops and maintains collaborative relationships with professional

partners

Links families with advocacy skills, resources, and information

Establishes and maintains relationships with community based

projects, organizations, and point persons

Meets on weekly basis with designated CMS office staff

Coordinates job responsibilities with designated area office CMS FHP

liaison

Participates in activities to develop their personal leadership skills

Maintains a knowledge of CMS Programs

Requirements:

Experience as a parent raising a child with special health care needs

Ability to communicate and listen effectively to families and

professionals from diverse cultures

Excellent Organizational skills

Ability to communicate and work with multiple discipline teams

Ability to work independently and make decisions

Self Starter

Promotes a positive attitude

Part Time Position Pays $10.00/hour
