Guest guest Posted May 16, 2001 Report Share Posted May 16, 2001 75-year-old white female recently hospitalized 4/27/01 to 5/3/01 with chronic renal deficiency, acute nonanuric renal failure, anemia with possible B-12 deficiency, Type 2 diabetes mellitus, UTI and hyperlipidemia. The patient was discharged on 5/3/01 and seen here on 5/4/01 and put on medications which include: instead of Prevpac was put on Biaxin 500 mg b.i.d., Amoxicillin 500 mg b.i.d. for 14 days, Prevacid 30 mg b.i.d. for 14 days, s/l " Metaclokamine " 10 mg a.c. . . .
