It cost me $3 for the ID at the DMV and $7 for the parking pass at the Tag agency. The DMV was wonderful and didn't make my son wait in the long line. We all know how difficult that can be. They walked us right in. It took 15 min. The hardest part was getting him to look at the camera for the picture. Make sure you bring the child's Birth certificate and social security card with you. the form can be downloaded from the state of florida department of highway and motor vehicles website - it is a .pdf file. There is also a fee to obtain a florida id ($15.00, I believe - call ahead to double check) and a picture is taken of your loved one at the license bureau. You cannot bring a picture of your own. They need your loved one there in person.

It is a good question. I was trying to get one as well, because I have two kids on the spectrum (ages 3 & 4), and they both like to act like "jell-o" every time we need to cross a street. I asked at the DMV, and was told that their pediatrician has to sign a form (which is also available at the DMV), and from my understanding, it's pretty specific. I figured I had no chance, so I didn't bother. Now I just park near the cart return area. Best of luck to anyone who's trying to get one though! :-)

Yes. All you need is an application for disabled person parking permit form signed by your childs doctor, verifying your childs disability. You will need to take your child to the DMV to have his or her picture taken for a Florida Identification card before the permit can be issued.The following web page should provide you with everything you need.
