The deadline for submitting nominations is approaching quickly. Nominations must be postmarked by September 15, 2001. Nominations are limited to: the nomination form (available in the magazine or at www <http://www.tdh.state.tx.us/hcqs/ems/AwardFrm.pdf> ..tdh.state.tx.us/hcqs/ems/AwardFrm.pdf), three typewritten pages of background info, one page of documentation or examples, and five letters.
