Kim,

I had 29.9 after surgery last year. My surgeon and Endo are Thyroid

Specialists, the surgeon having 25+ years experience and both are

highly regarded. Having said that, I didn't know of this site at the

time of my RAI - I would have asked more questions given the strong

medical opinions of the Doctors here.

I think it's a good idea to get feedback from this support group, but

keep in mind every member's pathology and operative report probably

reads differently and treatment is based on your individual

situation - can't base your situation on mine, for example.

Was 29.9 right or wrong? I'll know more after my scan on Oct. 12th.

I do think that after you've done your homework, there has to come a

time that you are comfortable enough with your own medical team to

trust their expertise - even if it differs from other Doctor's advice.

Their is no Thyca Commandment that says, " thou shalt not have less

than 100 mCi " !!

LOL. That is EXACTLY what my Nuclear Med Dr. says ( " thou shalt not

have less than 100 mCi " !!) He says he is not terribly scientific about

the dose of RAI, but he prefers to give 100mCi's if contained to the

thyroid bed, 150mCi's if spread to the lymphs and 200mCi's if distantly

spread. He doesn't give adults less than 100mCi's unless there is some

other situation that contraindicates that as a minimum dose.

Hi everyone! I am ready to have my first RAI. However, I am having

difficulty with the amounts the Doctors want to give. They all seem

to want to give only 29.9mci. One wants to give 29.9mci 2 days apart

(not 1time of 60). I thought it would be helpful if I took a little

survey from accross the country. I had a 4.5cm pap w/ foll variant.

The question for you to answer is.......

WHAT WAS THE AMOUNT OF RAI (mci)YOU HAD FOR YOUR FIRST TREATMENT

If they feel they got all of the tumor/nodes-my nuc med dept gives 100

mCi for a clean up job.

Hi Kim,

I had 30mCi Rai. My tumor was 2.2cm confined to right lobe and a 0.5 x 0.5

x0.4cm at the junction of the isthmus and right lobe. Nodes were negative. After

the scanning and with all the facts, my age(48), path report etc this is what my

nuclear med DR recomended, so I went with it. Only time will tell...

pap. thyca 11/00 (both lobes and extensive lymph nodes)

tt 12/00, RAD 175mCi 1/01

total body scan one week later with residual activity in the neck

TG 1.4 5 months after RAI. TG 2.4 3 months later that is current as of 2 weeks

ago

TSH 2.32, T3T4 1.4, TG antibodies <2, TG 2.4

my synthroid has been changed from .125 to .137 to try to suppress my TSH.

I had 150mci.

pap thyca follic variant. 1.1x.8 cm., no lymph node involvement.

no apparent mets on post-ablation scan (a week later).

due for 1 yr scan in a few weeks.

It's interesting that the latest (2001) AACE guidelines for Clinical

Practice/Management of Thyroid Carcinoma, the work of well over 113

contributing endocrinologists and Thyroid Cancer Specialists states:

" The standard I131 dose in the past for RRA was 75-150 mCi. In

recent years, some US Centers have used a low-dose regimen of 25 to

29.9 mCi, especially if the amount of thyroid remnant tissue was

small. ....Currently, no consensus exists about the most appropriate

dose of RAI for remnant ablation. "

The jury is obviously still out about this, even among the

specialists. I really wonder why it's so difficult to reach some sort

of professional consensus.
