Vaccine Excemption Forms???

Hi: Can someone guide me as to where I need to go to fill out the forms that I need to claim an exemption for my son? He is 4 and has all of his vaccines up to age 3 but we do not want to continue vaccinating him any longer. Thanks in advance. B

You just go to the health dept. -- tell them you need a " religious exemption form " ... it's really not hard. If anyone asks you why -- tell them they can't! It's against the law for them to question you. It's a blue form... they sign and you sign. Pretty quick actually.

Thanks...where is the health department in Broward? Will he also need a form from his pediatrician as well or not?
