We too are hospital based. Just a question here. Why would you need

to keep a copy of the transfer form. If you are an employee of the hospital,

then the form and copy of your records become a permanent copy of the patients

records. If at any time a issue arises, I am sure that your boss or the

hospital administrator would pull the records for review. There must be

more to this story. Are you sure it was TDH and not JCAHO that told them

that.
