I seams that a worker at a local boat shop was bitten on the finger by a rattle snake. Well, while my partner was taking care of the pt. I decided to go look at the snake and confirm it's type and size. The only problem with this is that the snake had been taken by one of this guys buddy's. After tracking him down, and finding that it was indeed a 24 " rattler, I asked him what he was going to do with the snake. He held the snake up in the air and said " LUNCH. Snakes have such bad eyesight, that is why they have the sensory pits (as in pit vipers). In the Navajo Nation, we rarely had snake bites because there was a GREAT respect for them, from a religious and societal perspective. The only bite I remember was a 2 YO Hopi girl who lived in Phoenix and was visiting her aunt. She was bitten by a 3 ft. Western Diamondback, which she stepped on while walking through brush. Great care... the Hopi EMS (a super bunch of medics!) had the kiddo in the IHS clinic within 20 minutes of the bite (roads are miserable) with positive species ID, bilat I Vs, O2, and baseline labs drawn en route from the scene. We were alerted prior to arrival of the child at the clinic and picked up the kid just as the antivenin was hung (at 30 min post bite). The single puncture site was on the superior lateral surface of the right foot. By the time we got the child to PHX Childrens Level 1 Trauma Center 2 hours post bite, the leg at the mid thigh had swollen twice its normal size. The child wound up having a fasciotomy in the TC, but recovered without permanent impairment. We didn't do the fasciotomy, because we could still Doppler the DP and PT. We lost the DP as we were en route from PHX Sky Harbor airport (10 min L & S ) en route PCH. Fasciotomies were within our protocols for loss of distal pulses in compartmental syndrome, snake bite and extremity burns. Thoracic fasciotomy was in protocol for circumferential chest burns with decreased compliance absent a pneumothorax. Suggestions: 1) No restricting bands or ice packs. 2) get baseline diameter at each joint and midshaft of the affected extremity, as well as quality of distal pulses and cap refill. 3) Splint extremity and position dependent, unless vascular compromise suspected. 4) If fasciotomy is allowed, do so for prolonged, or potential prolonged vascular compromise. Note: this worked for us because we had the protocols, and we were in a unique situation because of the location and transport times, even by air. Think long and hard before considering this as an additional procedure.
