I think that a lot depends on the neph, to be honest. The only time I was on

prednisolone was when I was initially diagnosed, where the protein loss in 24

hours was 2.5g. My last 24 hour urine collection showed 1.7g/24 hours, but

admittedly, it was around the time when my creatinine was tapering down after

the acute renal failure episode, so the neph wasn't too concerned for that

reason. I have to see him again in a month. Anyway, glad that your other results

are stable.

I jsut went to the neph yesterday for the first time in three months

and my protein went up to 0.85 (up from 0.57, but still down from

the 8 it was at initially). I was OK with this but my neph decided

to essentially threaten me with a return to prednisone if my body

keeps going in the direction. If the protein goes above 1.0 he'll

do it. He saw the crestfallen look on my face when he suggested it

and said that maybe he'd try cellcept instead. My question is what

is the level when most neph's put their patients on something to

drop the protein level?

I've never really had a problem with much more than 1 g per day proteinuria

myself, Ingrid. I'm sure many others on the group have and could comment

better than I.

However, I would say it varies. If the proteinuria isn't heavy enough itself

to cause problems, I would say they don't really do anything other than

prescribe either an ACE inhibitor or an angiotensin II receptor blocker,

both BP meds that also tend to lower proteinuria to some extent.

Generally-speaking, the benefits vs risk of steroids doesn't favour their

use until proteinuria is at or approaching 3 g per day. But as I said, it

seems to be very variable, and some nephrologists might put a patient on

them sooner. The thing with steroids is that it's not a good idea to be on

them for an indefinite period. So, what happens is that you go on steroids,

proteinuria goes down, you go off steroids, next follow-up or two

proteinuria comes back up - and it's like a yo-yo.

Hi Ingrid

My preteinuria was at about 3g last time. My neph has adjusted bp meds and

I have just done another 24 hr collection. My neph doesn't believe that my

case will benefit from steroids, but if you have had results in the past it

may be worth considering again.
