1st Annual RSD/CRPS Society Symposium Presented by the American Society for RSD/CRPS, Inc. (ASRSD/CRPS) Hosted and sponsored by Sacred Heart Hospital August 9-11, 2002 Sacred Heart Hospital 421 Chew Street town, Pennsylvania 18102 Symposium Information The $23.00 basic registration package includes Symposium and beverages only. Meals are the participants responsibility. The $35.00 inclusive registration package includes all programs presented throughout the weekend at Sacred Heart Hospital for non-ASRSD/CRPS Chapter members. Registration fee includes the Friday Meet and Greet, lunch buffet on Saturday, August 10th and daily beverage service. The $30.00 ASRSD/CRPS Chapter Member package is for local support group members throughout the country and includes the Friday Meet and Greet, lunch buffet on Saturday and beverage service during Symposium hours. Fee: Before July 6th , 2002 Basic Registration Package: $23.00 Non-Chapter Member: $35.00 Chapter Member: $30.00 Fee: July 7th, 2002 and after Non-Chapter Member: $40.00 Chapter Member: $35.00 Seating is limited to 200. Early registration is strongly recommended. Schedule of events will be released on or before May 31, 2002. Speaker information will be released August 10, 2002. Childcare will be provided during Symposium hours at Sacred Heart Hospital. Fees are $3.00 for early registration before July 6th, 2002 and $4.00 thereafter. Parents/guardians will be required to sign a release form. * If your child takes any medications, it is the Parents/guardians responsibility to return to the childcare site to ensure proper distribution of any substance. The RSD/CRPS Society Awards dinner will be hosted at Comfort Suites on Saturday, August 10th, 2002 from 7 p.m. to 11 p.m. This is a buffet dinner with 4 main courses, 4 vegetables, salad and dessert. Cost of the Awards dinner is $35.00 and registration must be postmarked on or prior to July 6th, 2002. Seating is limited to 90. Early registration is encouraged. Awards to be presented include:

ASRSD/CRPS has made arrangements for local hotels to block rooms for the Symposium including the evenings of August 9-11th, 2002. It is the participant's responsibility to make reservations for their own accommodations. Cost of room and meals (other than those previously stated) is not included in the fee for the Symposium. I-78 to town. Take Exit 2 onto US-22 (towards PA-33/PHILLIPSBURG/RT-122/ALPHA). Merge onto US-22. Take the PA-145 N/MACARTHUR RD exit. Turn LEFT onto ESTATES DR. Turn LEFT onto PA-145 S. Enter next roundabout and take 2nd exit onto HAMILTON ST/PA-145 N. Turn LEFT onto N 6TH ST/PA-145 N. From Baltimore and Southern Points: I-95 North to I-476 North towards Plymouth meeting. Stay on I-476 North when it becomes part of the PA Tunrpike, NE Extension. Go approximately 30 miles to exit 33, to US-22 exit (towards I-78/ALLENTOWN/PA-309/HARRISBURG). Keep RIGHT at the fork in the ramp. Merge onto US-22 E. Take the PA-145 S/MACARTHUR RD exit towards 7TH STREET. Merge onto PA-145. Enter next roundabout and take 2nd exit onto HAMILTON ST/PA-145 N. Turn LEFT onto N 6TH ST/PA-145 N. From Pittsburgh, Ohio and points West: I-76 E. Take the I-81/US-11 exit- exit number 16- towards CARLISLE/HARRISBURG. Keep right, then left on ramp. Merge onto US-11. Turn LEFT to take the I-81 N ramp. Merge onto I-81 N. Take the I-78 E exit towards ALLENTOWN. Merge onto I-78 E. Take the US-22 E exit- exit number 15- towards PA-9/PENNSYLVANIA TURNPIKE/PA-309 N/LVI AIRPORT. Merge onto US-22 E. Take the PA-145 S/MACARTHUR RD exit towards 7TH STREET. Merge onto PA-145. Enter next roundabout and take 2nd exit onto HAMILTON ST/PA-145 N.
