There are two databases on Insurance that can help you find out if any BCBS plans paid for the band. You can sort the databases by " provider " by clicking on that column header. I did see some listings for BCBS PPOs. My messages are taking so long to post, someone else may already have responded to you about the Insurance Databases... if so, sorry! Also, have you already looked through the FILES and LINKS section? There may be some helpful advice in dealing with your particular denial.
